Athletic Paperwork

The following links are for the documents the UIL requires us to have on file for all athletes participating in athletics at the secondary (grades 7-12) school level.

When filling out the paperwork, please fill it out for the 2016-2017 school year.  All forms must be completed each year.  Physicals are required for all incoming 7th, 9th, and 11th grade students.  Additional information for each form follows below.



Medical History and Physical Examination

Everyone must complete a medical history form each year they participate in athletics.  The parent should read each question and complete the form accurately.  Please give details to any "Yes" answer.  The parent and student should sign at the bottom of the form after reading the statements.  The medical history should be completed prior to getting the physical examination.

A physical examination is required for all athletes entering the 7th, 9th, and  11th grades.  A new physical or physician's release is also required for "Yes" answers to the questions indicated on the medical history form.

Acknowledgement of Rules

The parents and student should read all rules carefully and sign and date the document after completing the information on the form.  This form is required each year the student participates in athletics.

Steroid Agreement Form

This form states that parents and students understand the risks involved in using steroids and that the use of steroids is prohibited by the UIL.  This form must be filled out each year the student participates in athletics and must be signed by both the student and parent.

Concussion Acknowledgement Form
This form defines and explains what a concussion is.  It also details the return to play protocol established by Texas Education Code, Section 38.157. This form must be filled out each year the student participates in athletics and must be signed by both the student and parent.

Emergency Information Sheet

This sheet provides a representative of the school with information to be  used in the event emergency medical care is needed for your child and to contact you.  Please fill out the sheet completely and sign and date  in the space provided.  This form must be completed each year the student participates in athletics and must be signed by both the student and parent.

Drug Testing Permit

This permit must be filled out each year a student participates in athletics, cheerleading, drill team, or band.  It is kept on record by the nurses, separate from the above UIL required paperwork and is required for your child's participation.  More information about the district's drug testing policy may be found here.

This form is to educate parents of the possibility of sudden cardiac arrest (SCA) What causes SCA, the signs and symptoms, and treatment.
